Baccalaureate/MD Application

The following documents are required to be considered for admission to the Baccalaureate/MD program:

Both the USC Baccalaureate/MD Application and Part II Undergraduate Application for Admission must be completed online by December 1, 2009. The letter of recommendation and official high school transcript must be postmarked by December 1, 2009 and mailed to:

Please be sure to send a separate copy of your letter of recommendation and official high school transcript to the USC Office of Undergraduate Admission at the address noted on Forms 3, 4 and 7 of the Part II USC Undergraduate Application for Admission.  Please note that the letter of recommendation for the Part II USC Undergraduate Application for Admission does not have to be from a math or science teacher.
